PHP vs. IOP: Understanding the Difference in Outpatient Care
Written By
If you are exploring treatment options for yourself or a loved one, you have likely come across two terms that can be confusing at first: PHP and IOP. Both are forms of outpatient care — meaning you live at home rather than in a residential facility — but they differ significantly in terms of time commitment, clinical intensity, and who they are designed for.
Understanding the difference between a Partial Hospitalization Program (PHP) and an Intensive Outpatient Program (IOP) is one of the most important steps in finding the right level of care. This guide will break down both options clearly so you can make an informed decision.
A Partial Hospitalization Program is the most intensive level of outpatient care available. PHP is sometimes called “day treatment” because it functions similarly to an inpatient program during the day — you attend treatment for five to six hours per day, typically five days per week — but you return home each evening.
PHP is designed for individuals who need a high level of clinical structure and monitoring but do not require 24-hour residential care. It is commonly used as a step-down from inpatient or detox, or as a step-up for someone whose symptoms are too severe for standard IOP. At Engage Wellness, our Day Treatment (PHP) program provides comprehensive clinical support including individual therapy, group therapy, psychiatric evaluation, and medication management when appropriate.
An Intensive Outpatient Program is a structured treatment program that typically involves attending sessions three to five days per week, for three to four hours per session. IOP offers a meaningful level of clinical intensity while allowing significantly more flexibility than PHP — making it one of the most widely used levels of care for adults managing addiction and co-occurring mental health conditions.
IOP is appropriate for individuals who have completed a higher level of care and are ready to step down, or for those whose symptoms and life circumstances make IOP the right starting point. At Engage Wellness, our IOP includes individual therapy, group therapy, relapse prevention, and medication-assisted treatment (MAT) when clinically indicated.
| Feature | PHP (Day Treatment) | IOP |
|---|---|---|
| Hours per day | 5–6 hours | 3–4 hours |
| Days per week | 5 days | 3–5 days |
| Total weekly hours | 25–30 hours | 9–20 hours |
| Live at home? | Yes | Yes |
| Psychiatric oversight | Yes, typically daily | Yes, as needed |
| Best for | Step-down from inpatient; high acuity | Step-down from PHP; moderate acuity |
| Work/family flexibility | Limited | Moderate to high |
The decision between PHP and IOP is a clinical one — it should be made in collaboration with a licensed clinician who can assess your full history, current symptoms, and support system. That said, there are some general indicators that can help guide the conversation.
PHP may be the right fit if: you have recently completed a medical detox or inpatient stay and need a structured transition, your symptoms are severe enough that you need daily clinical contact, or you are at high risk of relapse without intensive daily support.
IOP may be the right fit if: you have completed PHP and are ready to step down, your symptoms are manageable with a few sessions per week, or you need to maintain work, school, or family responsibilities while still receiving meaningful clinical support.
At Engage Wellness in Acton, MA, we offer both PHP and IOP as part of a full continuum of outpatient care. This means that if you start in our Day Treatment program and are ready to step down to IOP, you can do so seamlessly — with the same clinical team, the same therapeutic relationships, and the same commitment to your recovery.
